Daarom heeft uw huisarts een Palliatieve Kit aangevraagd, die bij u thuis komt te staan. De Palliatieve Kit bevat medicijnen en hulpmiddelen die de huisarts of thuiszorg direct kan inzetten om deze klachten te verlichten.

De Palliatieve Kit staat verzegelt bij u thuis. Zo is de juiste zorg altijd binnen handbereik voor de huisarts of thuiszorgmedewerkers. Dat geeft rust en een veilig gevoel.

• Wat is de Palliatieve Kit? Dit is een box met medicijnen en hulpmiddelen. De middelen helpen om klachten zoals bijvoorbeeld benauwdheid, misselijkheid, pijn of plasproblemen snel te verlichten. U wordt hier niet sneller door opgenomen of overlijdt er niet eerder door.
• Waar bewaart u de kit? Niet bij de verwarming of in een vochtige ruimte, zoals de badkamer. Dit kan de houdbaarheid van de medicijnen beïnvloeden.
• Kosten Er kan een rekening van de apotheek komen, afhankelijk van uw zorgverzekering.
• Wat te doen bij onverwachte problemen of klachten? Uw huisarts spreekt met u en de thuiszorg af wie u bij onverwachte problemen moet bellen. Soms is dit uw eigen huisarts, soms de huisartsenpost. De huisarts of thuiszorgmedewerker bepaalt of er middelen uit de Palliatieve Kit nodig zijn. Ga niet zelf aan de slag met de kit.

• Where should you store the kit? Keep it away from heat sources and out of humid areas such as the bathroom, as this may affect the shelf life of the medications.
• Costs. Depending on your health insurance, you may receive a bill from the pharmacy.
• What to do in case of unexpected problems or symptoms? Your GP will discuss with you and the home care team who to call in case of unexpected problems.
